Financial Policies

Convenience Fee: A 3% fee will be changed if using a Visa, MasterCard, Discover, or American Express card. No fee is assessed if using a debit card, HSA card, or FSA card.

Payments and Late Charges: Unless other arrangements are approved by us in writing, the balance on your statement is due and payable when the statement is issued and is past due if not paid within 10 (ten) days of the due date. A $10 late fee will be charged to any past due balance and cannot be removed. The initial down payment, up to $1200.00, is non-refundable after 3 business days.

Missed Appointment Fee:  Missed appointments will be charged a fee of $100.00.

Returned Checks: There is a fee of $35.00 for any checks returned by the bank and any automatic payment that fails to complete its transaction. After one returned payment the office will only accept cash, credit, and/or money order.

Charges to Account: We have the right to cancel your privilege to make charges against your account at any time. Future visits would need to be paid at the time of service.

Oral Hygiene: Poor oral hygiene slows orthodontic progress and increases the risk of decay and decalcification while wearing braces. During orthodontic treatment, it is imperative that the patient continues routine visits to their general dentist. It is the patient’s responsibility to schedule a cleaning and cavity check every three to six months to maintain proper oral health. OMI Orthodontics reserves the right to remove orthodontic appliances early if the patient does not follow proper oral hygiene instructions. No discount to the contracted fees will be given for the early removal of brackets. Excellent oral hygiene, reduced sugar consumption, and being selective with what a patient eats will help speed orthodontic treatment, minimize decay and white spots (decalcification), and decrease the possibility of periodontal (gum) disease/problems.

Noncompliance: During Aligner Treatment, if replanning is needed due to non-compliance of aligner wear, a minimum Replan Fee of $1098.00, plus additional fees per aligner will be applied. OMI Orthodontics reserves the right to charge for excessive breakage, elastic noncompliance, hygiene noncompliance, missed appointments, and patient decision to change treatment plan. A call and text will be sent prior to the charge being applied. If over estimated treatment time due to breakage, missed appointments, missed payments, and/or noncompliance, additional charges will be added to treatment.

Past Due Accounts: If your account becomes past due, we will take the necessary steps to collect this debt. If the account is not paid in accordance with the financial arrangements made, you will pay all collection costs, including collection agency costs, but not limited to attorney’s fees and court costs. Sometimes unexpected circumstances occur that may place your account in a delinquent status. Please call us when you first experience issues that may make your payment late so we can make appropriate arrangements. However, if your account does become delinquent more than 1 cycle and you have not made previous arrangements, we will require that you enroll in automatic payments as a condition of these financial arrangements herein.

Insurance: Insurance is a contract between you and your insurance company. We are NOT a party to this contract, in most cases. We will bill your insurance company as a courtesy to you. Although we may estimate what your insurance company may pay, it is the insurance company that makes the final determination of your eligibility. You agree to pay any portion of the charges not covered by insurance. If your insurance company requires a referral and/or preauthorization, you are responsible for obtaining it. Failure to obtain the referral and/or preauthorization may result in a lower payment from the insurance company.
