Wear Time Is the Whole Treatment
Twenty to twenty-two hours a day, every day. Trays only move teeth while they are in the mouth, so a tray sitting in a bag at lunch is a tray doing nothing. Patients who treat wear time as approximate are the ones whose treatment stretches.
Trays change on the schedule Dr. Johnson sets, with periodic checks to confirm the teeth are tracking the plan. When they are not, the plan is revised early — that is what the checks are for.
Practically, that means building the trays into the day rather than treating them as optional. Out for meals and for anything other than water, back in immediately afterwards, and in overnight without exception. Patients who manage the full wear time generally finish close to the original projection; the ones who do not are the reason aligner treatment has a reputation for running long.
One habit worth adopting from day one: keep the previous set of trays until treatment finishes. If a current tray splits or disappears, going back a stage for a day or two is far better than leaving the teeth unretained while a replacement is made.
Aligners for Teenagers
Some teenagers manage aligners impeccably. Others genuinely cannot, and that is not a character judgement so much as an observation about the age. Be honest about which teenager you have, because braces will beat aligners worn part-time every single time.
Adults dealing with teeth that shifted after childhood braces are often the best aligner candidates of all: the movements are small, the motivation is high, and treatment can be short.
